Healthconnect Participants enhance patient safety and continuity of care by accessing critical information from a broad range of community providers.
Benefits include:

Quality
The HIE provides historical information on prior diagnoses and chronic diseases for better continuity of care. Patients receive higher quality care when their clinicians have a more complete medical picture.

Safety
The HIE informs clinicians on their patients’ active medications, allergies, and pre-existing conditions. More data means safer care delivered for patients.

Efficiency
The HIE gives providers a 360-degree view of their patients’ health. This allows clinicians to make more informed, efficient, and actionable decisions, right at the point-of-care.

Cost-Savings
The HIE facilitates more cost-conscious delivery of care. By leveraging HIE information, clinicians can reduce redundant and over-utilized services.

Hospitals & Health Systems
Healthconnect has 95% of the region’s hospitals and health systems signed on to the health information exchange. Hospitals leverage the HIE for care coordination throughout the community.
The following large health systems are participating in the health information exchange:

Physicians
Healthconnect enables physicians to deliver better, faster, and more cost-effective care to their patients.
We work with the following physician-led organizations:

Physician Practices
Healthconnect works with physician practices of all sizes – small, medium, and large. Participating physicians can share clinical information with hospitals, pull relevant information from ED visits, access laboratory and diagnostic information, and take and deliver patient referrals.

Independent Practice Association
Healthconnect works with Renaissance Physicians, the region’s largest independent practice association (IPA). With 1,300 primary and specialist physicians in the Renaissance network, these clinicians consume Healthconnect Notifications to coordinate care for their patients.

Accountable Care Organizations
Healthconnect works with ACOs to improve care coordination activities for their patients through value-based payment models. ACOs leverage Healthconnect data to deliver high-quality, cost-effective, and efficient care for patients.

Harris County Medical Society
Healthconnect has a long-term relationship with the Harris County Medical Society, the largest county medical society in the United States, with a membership of more than 11,000 physicians and medical students.
