Healthconnect facilitates clinical integration across the care continuum by connecting disparate electronic health record systems across the region’s hospitals, health systems, integrated delivery networks, community clinics, physician office practices, and service providers across Texas.
To connect all venues of care - integrating different electronic health record systems so that providers can easily share medical information across the community.
To operate a sustainable health information exchange that coordinates care for the community, improves overall population health, enhances health and wellness initiatives, reduces healthcare costs, and advances opportunities for long-term research and disease prevention.
What We Do
As a neutral third-party, Healthconnect facilitates the exchange of information across the provider community. From small physician office practices to large health systems, Healthconnect integrates with different electronic health record systems to enable the exchange of clinical information.
This is a telehealth program that looks to manage 911 emergency calls and reduce hospital emergency department usage by re-routing non-urgent/non-critical patients to community clinics. The program has been lauded nationally as a case study in improving the cost-effectiveness of care.
